Required Paperwork

There are a number of required forms that must be submitted for each swimmer who wishes to participate in the Lenoir City Swim Team program. Each form is linked on this page under Documents. Unless otherwise noted, ALL forms are due on/before the first day of practice. All forms must be returned to a LCST Board Member or Coach.
